Injecting is an Art

1. Medical Clearance & Annual Assessment Fee

• All new and existing clients of Injection Site are required to maintain current medical clearance in order to receive any services, membership packages, or treatments.
   
• A non-refundable $25 assessment fee is required for this clearance and must be paid to our intake nurse or to Injection Site, whether the assessment is conducted in-house or through our partnered telemedicine provider.
   
• The assessment is valid for one (1) year from the date of clearance. After one year, clients must renew medical clearance and pay the $25 annual assessment fee before continuing services or starting new memberships/packages.
   
• If a client is not medically eligible for a particular service, Injection Site may, at its sole discretion, apply payments as credit toward other treatments, or issue a refund and discontinue any future auto-pay charges.
   

2. Eligibility

• Memberships and packages are valid for clients of Injection Site.
• Clients must be 18 years of age or older.
   

3. PRP Microneedling Membership Package

• Includes one (1) PRP Microneedling session per month for six (6) consecutive months at $198/month.
• Upon completion of the six-month term, client receives one (1) complimentary session.
   

4. Payment Terms

• Monthly membership or package payments, at the agreed-upon price, will be automatically charged to the client’s designated payment method on file.
   
• All payments are final and non-refundable.
• If a scheduled payment is declined, rejected, or returned unpaid, the client authorizes Injection Site to reattempt the charge and assess a $25 failed payment fee.
• Membership or package benefits may be temporarily suspended until outstanding balances are paid; however, automatic withdrawals will continue for the remainder of the commitment term.
   

5. Credits & Substitutions

• If a client does not use their monthly PRP Microneedling session, a credit equal to the session value may be applied toward another treatment of equal or greater value.
• Credits cannot be redeemed for cash; any price difference must be paid by the client.
   

6. Expiration

• Membership sessions and credits must be used within the six-month membership period.
• Unused sessions or credits expire at the end of the membership term and cannot be carried forward.
   

7. Transferability

• Memberships, sessions, and credits are non-transferable and may only be used by the purchasing client.

8. Scheduling & Appointment Cancellations

• Appointments must be scheduled in advance and are subject to availability.
• 24-hour notice is required to reschedule or cancel.
• Late cancellations or missed appointments will result in forfeiture of that session.
   

9. Membership & Package Commitment

• Memberships and packages are commitments for the full term agreed upon at purchase and cannot be cancelled, paused, or refunded once purchased.
• Clients are responsible for all payments for the full term, regardless of usage.
• At the sole discretion of Injection Site, any unused payments may be applied as credit toward other services offered by Injection Site.
   

10. Medical Eligibility

• All treatments are subject to medical assessment and clearance as described in Section 1.
• If a client is determined medically ineligible for PRP Microneedling, credit may be applied toward other treatments available at Injection Site, or membership payments may be refunded/discontinued at the sole discretion of Injection Site.
   

11. Changes to Terms

• Injection Site reserves the right to update, modify, or change these Terms & Conditions, membership or package details, pricing, and policies at any time, with or without prior notice.
• The most current version of the Terms & Conditions will always apply and will supersede any prior versions.
